Current clients may obtain a Records Request Form by sending a secure message through the client portal. Once received, a Records Request Form will be provided electronically through the portal. After the completed form is submitted, the request will be processed.
Former clients no longer have access to the client portal. To request records, please download and complete the Records Request Form. Completed forms may be submitted by fax, email, or mail for processing.
